To fulfill all the requirements set forth by the Graduate College you will need to complete several forms before you complete your graduate program.
Form 1
The first step is to establish a Graduate Committee. This committee consists of at least three members of the graduate faculty. It must include two members, including the major professor, from inside the major or program. One member of the committee must be from outside the student’s field of emphasis. It is ideal to complete this form in your 3rd or 4th semester of the program, but no later than the semester before you plan to graduate.
Access Form 1 (Graduate Committee). Use the following information to fill in the top portion of the form:

Form 2
The next form you will need to complete is the Program of Study form. This form lays out your plan of coursework you plan to use to complete your graduate degree. You will need to establish your Graduate Committee before you can complete your Program of Study. It is ideal to complete this form in your 3rd or 4th semester of the program, but no later than the semester before you plan to graduate.
Access Form 2 (Program of Study). Use the following information to fill in the top portion of the form:

It is essential your fill in 13b and indicate your Final Examination or Projected
Graduation Semester/Year.
Form 3
The semester in which you plan to graduate, you will need to submit an Application for Graduation. This is generally due the first Friday of the semester.
Access Form 3 (Application for Graduation). Use the following information to fill in the top portion of the form:
Degree: Master of Family and Consumer Sciences
Indicate one: Non-thesis
Program or Department: FCS
Major: FCS
Area of Specialization:
Family Financial Planning
Family Financial Planning Certificate
Gerontology
Gerontology Certificate
Major Professor: Use the individual you selected in Form 1 above.
Form 4
The semester in which you plan to graduate, you will work with your Major Professor and Graduate Committee to complete your Request for Final Oral. This form is not an online form and must be obtained from your Major Professor. This form must be completed and returned to the Graduate College no later than 3 weeks before you plan to have your final oral exam.
