- Awards. Nomination forms and guidelines for awards administered by the College of Human Sciences.
- Purchasing Cards Receipts. Submit this form when turning in P-card receipts to the Dean's Office.
- Vacation/Sick Leave. Submit this form when turning in vacation and/or sick leave to the Dean's Office.
- New/Leaving Faculty/Staff Notice. Submit this form to notify the Dean's Office of a new faculty/staff member and to ensure they are included in appropriate CHS databases and communications. This form is also used to notify the Dean's Office of the end of a faculty or staff member's employment.
